Specialist Services
Throughout out its time Way Ahead have been asked to both develop and implement specialist services working partnership with the local authority to help provide alternative services for people within the local community.
The Out of Hours Service
This scheme works in partnership with Social Services, The Primary Care Trust and Deane Help Line.
A team of carers provide care throughout the night in the event of an emergency or to help with ongoing packages throughout Taunton Deane. This could be planned visits to turn a Service user to prevent bed sores, catheter maintenance, prevent care home/Hospital admission or to assist the District Nurse in a double up situation.
this then allows a service user to stay in their own home for longer rather than be admitted into a residential or nursing home or be subject to continual hospital admissions
The Rapid Response Service
Initially a pilot scheme throughout Taunton Dean this scheme was set up to provide immediate care and support to hospital discharges instead of blocking vital hospital beds.
Rather than a Service Users staying in hospital and blocking valuable beds the Rapid Response team provides the essential care required for 2 weeks whilst the Social Services Care Co-ordination team organise permanent provision from one of the eight block provider throughout Somerset
Based at Musgrove Park Hospital in Taunton Way Ahead's Rapid Response team work very closely with both the Musgrove Social Work and Social Services Co-ordination Teams to ensure a smooth admission from the hospital for the Service User.
The Waking Night Service
Working in partnership with Social Services, Way Ahead also provided a Waking Night Service (WNS) 7 days a week throughout Taunton Deane from 10pm to 7am This services is designed to provide short-term personal care at home.
Way Ahead will provide one fully trained member of staff to facilitate the needs of both the Service User and their carer throughout the night. Liaising with both the Service User and the primary Carer on the night Way Ahead ensure that the service provided is seamless and supports any service already in place.
The main objectives of the WNS is to provide short term support and assess the need for further on-going support from either the Out of Hours Service or regular waking night cover from a Block Care Provider
